A Strategy for Pharmacists¡¯ Participation in an Antimicrobial Stewardship Program and Its Effect

Abstract
Backgrounds : Recently, the role of pharmacists in Antimicrobial Stewardship Program (ASP) has been emphasized and its effect has been reported. However, it is difficult for pharmacists to be actively involved in Korea mostly due to limited human resources. Therefore, a pharmacy intervention program for ASP was designed in this study to determine its effects on appropriate antibiotic use.

Methods : One pharmacy resident and one clinical pharmacist participated in the ASP in Seoul National University Bundang Hospital. They monitored the dosage of antibiotics, drug interactions, and premedication to prevent adverse effects such as premedication therapy of amphotericin B for 30 minutes daily using a computer program in the hospital. Retrospective chart review was conducted to compare the appropriateness of the selected antibiotic use before and after implementation of the pharmacy intervention program for ASP.

Results : No significant difference was found in the proportions of patients prescribed with inappropriate antibiotics before and after the program. However, the proportion of inappropriate prescriptions was reduced from 23.8% to 9.7%. The proportion of inappropriate prescriptions in which pharmacists intervened was increased from 8.6% to 20.7%. Seventy-five percent of the interventions were conducted by two pharmacists involved in this program. Twenty-five percent of the interventions were conducted by the pharmacists not involved in this program.

Conclusion : Appropriateness of antibiotic use was improved after implementation of the pharmacy intervention program for ASP. Therefore, ASP could be effectively applied in Korean hospitals.
